Extendicare operates the home that Ontario Health at Home barred from new admissions effective August 6, 2026, on the ministry's stated belief of "risk to the health, safety, and well-being of residents." The home has been inspected 18 times in two years with 120 findings of non-compliance. This is the second such order — a six-month order was issued in December 2024 after 41 infractions and lifted in June 2025.
